Welcome to your 2023 Safe Harbor Benefits
Helping you and your family reach and maintain good health is very important to Safe Harbor Insurance. We are pleased to offer a selection of benefits designed with your health and wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Safe Harbor Insurance. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and child(ren) up to age 26.
Eligibility & Qualifying Events
The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.
EMPLOYEE ELIGIBILITY
You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 30 days from your date of hire.
***Reminder:
You have up to 30 days after your benefit effective date to make your elections. If you wait until the 31st day, you will need to wait until Open enrollment or a qualifying event.
DEPENDENT ELIGIBILITY
You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.

You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Anthem will continue to be the medical and prescription carrier this year, with the changes below on the current plan design.
In addition, if you need a specialty medication, you can contact IngenioRX, which will support you with:
- scheduling delivery times and locations
- finding financial assistance options
- assisting with prior authorizations

Virtual Visits
WHEN CAN I USE A VIRTUAL VISIT?
When you have a non-emergency condition and:
- your doctor is not available;
- you become ill while traveling;
- When you are considering visiting a hospital emergency room for a non-emergency health condition.
*Your covered children may also use Virtual Visits when a parent or legal guardian is present for the visit.

- Bladder infection
- Seasonal flu
- Sinus
- Sore throat
- Stomach
- Rash
- Bronchitis
- Diarrhea
- Fever
- Pink eye
HOW DOES IT WORK?
- To request a telehealth visit, log in to your Anthem account and choose “Virtual Visit.” You can also download the SydneySM Health mobile app. The Sydney app will connect you to a doctor through a virtual chat session or a virtual video session.

Health Savings Account ( HSA)
A Health Savings Account (HSA) is a tax-free savings account is owned by you, is 100% vested from day one, and let’s you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.
UNDERSTANDING YOUR HSA
- Pre-tax contributions are deducted through payroll and deposited into your HSA account
- You can use your HSA available funds to pay for qualified medical expenses tax-free
- HSA funds can be used for non-eligible expenses, but will be subject to regular income taxes and a 20% excise tax penalty
- Unused funds remain in your account for future use and roll over each calendar year
- HSAs remain with you even if you change health plans or companies. If you open an HSA and later become ineligible to make contributions, you can still use your remaining funds
- You can change your HSA contribution at any time during the plan year for any reason.

- Online – The most convenient way to apply. Just go to key.com/hsa and choose “Apply Online.” The easy-to-follow instructions will guide you through the process.

Dental Benefits through Anthem provide comprehensive coverage to help you and your family maintain good dental health. Your coverage will be greater if you go to an in-network provider.

Safe Harbor offers vision coverage through Lincoln to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Please note: Members may choose between prescription glasses (lenses and frame) and contact lenses, not both.
You have many choices when it comes to using your benefits. As a Blue View Vision plan member, you have access to one of the nation’s largest vision networks. You may choose from many private practice doctors, local optical stores, and national retail stores including LensCrafters®, Target Optical®, and most Pearle Vision® locations. You may also use your in-network benefits to order eyewear online at Glasses.com and ContactsDirect.com. To locate a participating network eye care doctor or location, log in at anthem.com, or from the home page menu under Care, select Find a Doctor. You may also call member services for assistance at 1-866-723-0515.
Blue View Vision Plan
Routine Eye Exam: $10 copay
Eyeglass Frames: $130 allowance, then 20% off any remaining balance
Eyeglass Lenses: $10 copay
Contacts: ( Instead of Glass Frames) $130 allowance then 15% off the remaining balance ( Non- disposable) $130 allowance for disposable

401(k) and Roth(k) Plans
401(k) Plan
Safe Harbor Insurance Agency encourages employees to save for their retirement and provides a company-sponsored 401(k) plan. The 401(k) plan offers a company-matching contribution to help employees with their retirement savings. Safe Harbor Insurance Agency matches 100% of the first 3% deferred by employees and 50% of the next 2% deferred. The entry date is the first of the month after 90 days of service for deferral and matching contributions, and 1 year of service for profit-sharing contributions. All participants contributing less than 4% will automatically increase each year by 1% until the 6% cap is met. Employees can opt out if they choose not to be part of the Step-Up program.

As a reminder, the Company’s contributions are deposited into your account each pay period and employees are ___% vested immediately. For 2023, you may contribute up to $22,500 for the calendar year. If you are over age 50, you can contribute an additional contribution known as a “catch-up” contribution up to $7,500 for the current calendar year.
Roth(k) Plan
Safe Harbor Insurance Agency has a Roth(k) option. Roth(k) is an optional feature that allows you to make after-tax contributions in addition to, or instead of, pre-tax contributions. As long as you stay within the plan’s limits, you may make either 401(k) pre-tax contributions or Roth(k) after-tax contributions or a combination of both. All your contributions are treated the same relative to other plan provisions, such as employer match, loan availability, and hardship withdrawals. If you are currently enrolled in the 401(k) Plan and would like to elect the Roth(k) option, you can log onto the website below or call 1-800-523-1188.
Employee Contributions
Pre-Tax Contributions
You share in the cost of your Medical, Dental and Vision Benefits. Your employee contributions for these benefits are taken out of your pay on a pre-tax basis as set forth below. In addition, if you elect to contribute to a flexible spending account or a health savings account, those contributions will be taken out of your pay on a pre-tax basis based on your election.
After-Tax Contributions
If you choose to purchase Supplemental Life insurance and Optional AD&D insurance benefits for yourself or your dependents, your contributions will be taken out of your pay on an after-tax basis. You will be able to see the costs for these benefits in the enrollment system when you log in.
